- ItemAfrontamiento diádico, tipos de estresores y etapa del ciclo de vida en personas que están en una relación de pareja en Chile(2024) Rodríguez Labarca, Macarena del Pilar; Rivera, Diana; Pontificia Universidad Católica de Chile. Escuela de PsicologíaLa relación entre estrés y afrontamiento diádico (A.D) es área de gran desarrollo, existiendo amplia evidencia sobre su relevancia en la calidad y bienestar de las relaciones de parejas. Los desafíos en este campo son el poder estudiar la evolución y cambios que puede enfrentar el A.D a través del tiempo, considerando el ciclo de vida y sus particularidades contextuales. En este contexto, la presenta investigación se ha planteado poder establecer la relación entre estresores externos e internos y el A.D considerando la etapa de vida de las personas. Se trabajó con datos secundarios proyecto Fondecyt, considerando una muestra de 394 personas, definiéndose tres etapas de vida a estudiar. Se utilizó un diseño de investigación no experimental, transversal y correlacional, Los resultados indican que las personas en pareja con hijos en infancia temprana, presentan mayores niveles de estrés, tanto interno como externo a la relación, siendo el estrés interno el de mayor influencia en las parejas en todas las etapas. En consideración a la relación entre A.D y estrés según etapa del ciclo de vida, los resultados dan cuenta que la etapa de vida no modera la relación entre variables, observándose una relación inversa entre el estrés y afrontamiento diádico, existiendo un A.D menor en etapas posteriores de vida, cuando se cuenta con hijos mayores y mayor tiempo de relación.
- ItemAn in vitro model for photosynthetic cancer treatment: a study of photosynthetic and tumor cell co-culture(2024) Holmes Videla, Christopher Edward; Egaña, José T.; Pontificia Universidad Católica de Chile. Escuela de IngenieríaLa baja tensión de oxígeno (hipoxia) en tumores conlleva a la resistencia de una serie de tratamientos clínicos, por lo que el uso de microorganismos fotosintéticos se ha planteado recientemente como una alternativa segura para proveer de oxígeno al microambiente tumoral. En este trabajo se estudió la biocompatibilidad de la microalga Chlamydomonas reinhardtii con condiciones de crecimiento de células mamíferas. Se cultivó esta microalga junto con células de melanoma, con ambos tipos celulares resultando biocompatibles entre sí, pero con las microalgas afectando la organización del citoesqueleto de las células tumorales. Se observó que la oxigenación en co-cultivos era suficiente para eliminar condiciones de hipoxia y producir más oxigeno del requerido metabólicamente por las células tumorales. Se observó que esta producción de oxígeno era capaz de aumentar la muerte tumoral por radioterapia. Como último objetivo se estableció un modelo tumoral fotosintético en 3D por medio de una matriz de colágeno. Se estudiaron las interacciones entre ambos tipos celulares con la matriz junto con la oxigenación de células tumorales en un ambiente 3D hipóxico. Se demostró que ambos tipos celulares son biocompatibles por al menos 24 horas de contacto en condiciones de cultivo celular y que una razón de 10:1 microalgas a células, era capaz de cumplir con los requerimientos metabólicos de las células tumorales, produciendo un exceso de más del doble del oxígeno requerido, el cual puede ser usado a futuro para terapias antitumorales. Este trabajo muestra ue el uso de Chlamydomonas reinhardtii es una alternativa viable para la oxigenación de tumores para tratamiento clínicos, mientras que el modelo tumoral fotosintético in vitro presentado nos entregó un simple recurso para estudiar los efectos de la fotosíntesis en el microambiente tumoral y una herramienta para manipular esta aproximación para optimizar su uso en terapias clínicas.
- ItemAnálisis de la evidencia disponible sobre las estrategias de los programas odontológicos en Atención Primaria de Salud y su influencia en la calidad de vida del adulto mayor. Facultad de MedicinaEl envejecimiento poblacional ha generado una visión distinta en cuanto a las políticas públicas realizadas, ya que deben crearse programas dirigidos especialmente a los adultos mayores y la importancia de entregar una mejor calidad de vida. En el área de la salud, Chile cuenta con programas de salud integral para las personas mayores, pero desafortunadamente ha sido insuficiente en el área de salud oral ya que la cobertura odontológica para la población de adultos mayores se restringen para atenciones odontológicas limitadas. El objetivo de este trabajo es analizar la evidencia publicada en torno a las estrategias en los programas odontológicos en Atención Primaria de Salud y su influencia en la calidad de vida del adulto mayor. Se realizó una revisión tipo scoping review de estudios entre los años 2001 y 2022. La búsqueda de los artículos se realizó en plataformas digitales: Pubmed, SciELO y Scopus, seleccionando 14 artículos cuyos resultados indican la relación existente entre el estado de salud bucal con la calidad de vida relacionada con la salud oral (OHRQoL) en los adultos mayores.Del total de artículos seleccionados, el 35,7 % de los estudios evidenció un aumento en la calidad de vida autopercibida con la intervención odontológica aplicada . De los 14 artículos que cumplieron los criterios de inclusión, el 35,7% corresponden a intervenciones odontológicas y 64.3 % corresponden a estudios para determinar la relación de factores sociodemográficos y parámetros clínicos como factores predictivos de la calidad de vida relacionada con la salud oral. El conocimiento relacionado con la salud bucal en los adultos mayores y su influencia en la calidad de vida es limitado. Los estudios incluidos informaron una variedad de indicadores de salud bucal clínicos objetivos y subjetivos, lo que indica una falta de medidas estandarizadas de enfermedades bucales entre adultos mayores. A medida que la población geriátrica continúa aumentando, los modelos actuales de atención odontológica deben reevaluarse para garantizar la satisfacción de las necesidades de salud específicas de los adultos mayores. El concepto de calidad de vida incorpora esencialmente la autopercepción del paciente como una necesidad en la evaluación de resultados en salud, es esencial un fortalecimiento de las estrategias utilizadas en los programas odontológicos actuales, con la definición de nuevos programas o una reformulación de los ya existentes para dar cobertura a las necesidades nacionales en salud oral en la Atención Primaria de Salud de este grupo poblacional.Actividad Final Integrada para Optar al Grado de Magíster en Salud Pública
- ItemAssociation between the brain injury location and its volume with the severity profile of language impairment measured with the ScreeLing in Chilean adults after stroke: a pilot study(2024) Urrutia Goldsack, Francisca Pilar; Mendez Orellana, Carolina; Pontificia Universidad Católica de Chile. Escuela de MedicinaIntroduction: Post-stroke aphasia is a high-prevalence disorder that impacts an individual's quality of life, environment, and health care systems. Thanks to novel neuroimaging techniques, new cortical language processing models have emerged; however, characterizing the neuronal organization associated with a linguistic task and domain has proven to be surprisingly challenging. This study aims to explore associations between the brain injury location and extension with the severity profile of language impairment measured with the Chilean version of the ScreeLing in post-stroke adults.Methods: This is an analytical, observational, cross-sectional study. We recruited 144 post-stroke adults within the first week after stroke. Neuroimaging and language performance data were collected. Our dependent variable was the ScreeLing score, while the brain injury location and volume were the independent variables. Linear regression models were performed in RStudio using a significance level of p < 0.05.Results: Among the patients, ScreeLing diagnosed 56 with aphasia and 60 without linguistic disorders, while 28 could not be included in the analysis. In the aphasic group, linear regressions revealed an association between the left temporal lesion and phonological performance; subcortical lesions correlated with lower phonological and syntactic scores. Poorer performance in the overall ScreeLing was associated with injuries in the left temporal lobe and subcortical regions. No association was found between the ScreeLing performance and the extension of the lesion.Conclusions: This study corroborated associations described in modern language processing models. Our future efforts will focus on precise location of the lesion and expand the sample size evaluated with MRI.
- ItemCampaña ¡Cuida tu salud mental! 2023: intervención psicoeducativa sobre atención plena, autocompasión y gratitud como hábitos de vida saludable dirigida a la población chilena(2024) Pereira Hernández, Fernanda Paola; Errázuriz Arellano, Paula; Pontificia Universidad Católica de Chile. Escuela de PsicologíaLa primera versión de la campaña “¡Cuida tu salud mental!” 2021 surge como respuesta a la necesidad de abordar la crisis de salud mental en Chile, principalmente a raíz de la pandemia COVID-19. Adicionalmente, en redes sociales abunda información de salud mental, pero la presencia de desinformación dificulta el acceso a datos fiables, afectando la toma de decisiones de los usuarios. Por otra parte, los estilos de vida relacionados con comportamientos de riesgo son responsables de aproximadamente el 32% de las defunciones en el país, subrayando la importancia de difundir conocimiento asociado a prácticas saludables para fortalecer la salud mental y promover el bienestar a través de contenido de calidad, con respaldo empírico, y de fácil acceso a los usuarios.En el presente trabajo se expone la creación, desarrollo y evaluación del material psicoeducativo para la tercera versión de la Campaña ¡Cuida tu salud mental! dando continuidad al proyecto de los años 2021 y 2022. Esta tercera versión se enfoca en la creación de material psicoeducativo orientado a la promoción de hábitos saludables, tales como: prácticas contemplativas, movimiento corporal, alimentación saludable y buen dormir. De las cuatro temáticas, aquí se abordarán tres prácticas contemplativas, conformadas por atención plena, autocompasión y gratitud. Se destaca la importancia de considerar estas prácticas como hábitos saludables, resaltando sus beneficios en torno a la prevención y promoción de salud mental. Para asegurar la claridad, calidad y utilidad del material psicoeducativo, se diseñó una propuesta de guión, basado en una revisión bibliográfica sobre prácticas contemplativas. Esta propuesta fue sometida a revisión por parte de 18 profesionales expertos en salud mental y hábitos saludables, y de 63 participantes representativos del público objetivo. Se emplearon cuestionarios de preguntas Likert y abiertas, y se realizó un análisis cualitativo y cuantitativo de cuatro categorías principales. Las categorías fueron: evaluación global del material, calidad del contenido, utilidad de las estrategias entregadas, y recomendaciones para mejorar. En cuanto a los resultados, el promedio de respuestas en ambos grupos (profesionales y usuarios) fue superior a 6 puntos, de un máximo de 7. Un mayor nivel educativo en usuarios predijo de forma significativa una mejor puntuación asignada en cuatro ítems. También, la identificación de los participantes usuario con el género femenino predijo una mejor puntuación en un ítem específico. En base a los puntajes y comentarios recibidos, se realizaron ajustes en el material psicoeducativo, relacionados con la simplificación del lenguaje e inclusión de ejemplos sobre prácticas contemplativas. En coordinación con instituciones asociadas: la fundación PsiConecta, el Instituto Milenio para la Investigación en Depresión y Personalidad (MIDAP), profesores expertos de la Pontificia Universidad Católica de Chile, el apoyo de Metro de Santiago y la agencia de publicidad Loca Santiago se procederá a la difusión del material oficial de la campaña, con la expectativa de proporcionar información clara y promover el bienestar asociado a las prácticas saludables beneficiosas para la salud mental.
- ItemCampaña ¡Cuida tu Salud Mental!: intervención psicoeducativa sobre el buen dormir como hábito saludable para la salud mental(2024) Canales Sánchez, Javiera Valentina; Errázuriz Arellano, Paula; Pontificia Universidad Católica de Chile. Escuela de PsicologíaEl presente trabajo se enmarca en la campaña "¡Cuida tu Salud Mental! Intervención psicoeducativa sobre el buen dormir como hábito saludable para la salud mental", resaltando la relevancia global de la salud mental, con aproximadamente el 13,04% de la población mundial viviendo con algún trastorno mental, especialmente depresivos y ansiosos (Institute for Health Metrics and Evaluation [IHME], 2019). En el contexto de Chile, el 15,66% de la población presenta trastornos mentales, siendo la depresión y ansiedad los más prevalentes (IHME, 2019). Ante esta problemática, la campaña “¡Cuida tu Salud Mental!" 2023, ha implementado una estrategia de prevención y promoción de la salud mental, enfocada en hábitos saludables y considerando determinantes sociales. El trabajo forma parte de la campaña "¡Cuida tu salud mental! 2023", impulsada por Fundación PsiConecta, Instituto Milenio para la Investigación de la Depresión [MIDAP] y la Pontificia Universidad Católica de Chile, Metro de Santiago y apoyada por la productora "Loca Santiago". Se propusieron hábitos para regular el ritmo circadiano y mejorar la calidad del sueño, los cuales fueron entregados a través de material psicoeducativo en la plataforma web de la campaña y pantallas de Metro de Santiago. La metodología de investigación involucró una revisión de la literatura y la creación de material psicoeducativo relevante por medio de la retroalimentación de profesionales y usuarios que evaluaron el producto para la construcción de los guiones de la campaña. Este trabajo es el resultado de un esfuerzo colaborativo de profesionales en psicología comprometidas con la promoción de hábitos saludables para la salud mental.
- ItemCaracterización de indicadores productivos de los terneros de la raza maremmana en el centro genético de Alberese, Italia, durante 32 años (1982-2013)(2024) Silva Pérez, Juan Eduardo; Larraín Prieto, Rafael; Pontificia Universidad Católica de Chile. Facultad de Agronomía e Ingeniería ForestalThis study examines productive indicators of Maremmana breed calves at the Alberese genetic center in Italy, over 32 years (1982-2013). In response to the challenges of climate change and risks to native breeds, were analyzed indicators such as average Birth Weight (BWp), average Weaning Weight (WWp), Weaning Weight adjusted to 205 days (WW205), Total Weaning Weight (TWW), Average Daily Weight Gain (ADGp), the Male and Female ratio (M/F), and the percentage of Twins (%T) were analyzed. The effect of variables such as y(Y), Birth Date (BD), Weaning Date (WD), First Calving Date (FCD), and Last Calving Date (LCD) on these indicators was evaluated over the analyzed period. With 3,313 calves born, means, medians, standard deviations, and minimum and maximum values were analyzed, using SAS/STAT 9.4 for linear regression statistical analysis. The findings show a reduction in BWp, and increases in AWW, WW205, ADGp, and TWW, the latter due to more animals weaned; the M/F ratio was maintained, and a significant increase in the %T was found. When analyzing the data in relation to the breed average, the indicators within the expected range for the breed include BWp, M/F, and %T, while the ADGp was below average and the WWp was above. The most relevant variables for the model were Y, LCD, BD, WD, and FCD in decreasing order, indicating adaptations and the influence of management on productivity and genetic conservation in the face of environmental changes.
- ItemCaracterización de los sistemas de información de la Red de Atención Primaria ANCORA UC(2023) Méndez Lagunas, Tomás; Soto D., Mauricio; Pontificia Universidad Católica de Chile. Departamento de Salud PúblicaObjetivos: caracterizar y analizar el grado de madurez de los sistemas de información en salud (SIS) de la red ANCORA UC, utilizando la herramienta IS4H-MM (Information Systems for Health – Maturity Model). El objetivo secundario es evaluar la aplicabilidad de esta herramienta en la Atención Primaria de Salud (APS) y su potencial como estándar diagnóstico en el nivel primario. Métodos: se aplicó la herramienta IS4H-MM, adaptada a la Red ANCORA UC, para evaluar los SIS en cuatro áreas clave: Gestión de Datos/Tecnologías de la Información, Gobernanza, Intercambio de Conocimiento e Innovación. La metodología incluyó la adaptación del cuestionario estructurado IS4H-MM para ajustarlo al contexto de ANCORA UC, junto con reuniones virtuales para una evaluación detallada y calificación de cada componente. Resultados: los principales hallazgos revelaron avances significativos y áreas de mejora en las cuatro dimensiones evaluadas. Se destacó la recolección sistemática de datos de salud, una estructura de gobernanza formal, mecanismos básicos para la gestión del conocimiento y familiaridad heterogénea con los conceptos de IS4H. Se identificaron oportunidades de mejora en la monitorización sistemática de la calidad de los datos, en la gestión del conocimiento y la innovación. Conclusiones: el estudio demostró la eficacia de la herramienta IS4H-MM en la evaluación de los SIS en la APS. Al ser adaptada, la herramienta IS4H-MM podría servir de estándar diagnóstico para la madurez de los SIS en APS. La ampliación de este enfoque evaluativo a otros centros es esencial para optimizar el sistema de salud en Chile.
- ItemControl of motivation for palatable food by kappa opioid receptor signaling and the neuropeptide Orexin-A(2023) Sandoval Caballero, Carolina Loreto; Pérez Leighton, Claudio; Pontificia Universidad Católica de Chile. Facultad de Ciencias BiológicasEating is a motivated behavior that provides nutrients to allow life. Animals evolved to perceive food as a positive reinforcer, resulting in increased motivation for food when the animal is hungry and decreased motivation when it is satiated. Motivation evolved to seek and eat food in an environment of food scarcity; however, it can become maladaptive in an environment with plenty food access, favoring excessive eating, weight gain, and obesity. Understanding the neuronal mechanisms regulating food motivation could improve therapies against obesity.Opioids and the hypothalamic Orexin/Dynorphin neurons are part of the brain mechanisms that regulate food intake and motivation for food. Opioids bind mainly to three G-protein coupled receptors (kappa, mu, and delta), which are expressed in brain regions that regulate food intake and motivation. Still, the contribution of each opioid receptor to food intake and motivation for food is unclear. The Orexin/Dynorphin neurons co-release the excitatory neuropeptides Orexins (e.g., Orexin-A and Orexin-B) and the inhibitory opioids Dynorphin (e.g., Dynorphin-A 1-13) into different areas, including the Oxytocin neurons from the paraventricular hypothalamic nucleus. The effects of Orexin and Dynorphin peptides in eating behavior are brain-site specific. For example, in the paraventricular hypothalamic nucleus, Dynorphin-A1-13 increases and Orexin-A decreases palatable food intake (i.e., tasty food, generally high in sugars and fats). Still, the opposite effect is observed in the ventral tegmental area. Dynorphin-A1-13 binds mainly to the kappa opioid receptor, which is expressed in the anorexigenic oxytocin neurons of the paraventricular hypothalamic nucleus; however, whether inhibition of oxytocin neurons is involved in the orexigenic effects of Dynorphin-A1-13 in the paraventricular hypothalamic nucleus is unknown. Also, in this brain region, Orexin-A blocks the orexigenic effects of Dynorphin-A1-13. Still, it is unclear whether these effects are mediated by changes in motivation for palatable food and whether the endogenous kappa opioid receptor signaling in the paraventricular hypothalamic nucleus regulates motivation for food. We hypothesize that in the paraventricular hypothalamic nucleus, Dynorphin-A1-13 signaling through the kappa opioid receptor promotes motivation for sucrose, and Orexin-A inhibits this behavior. We evaluated this hypothesis through four specific aims. [1] To determine through a meta-analysis the effects of kappa, mu, and delta opioid receptor ligands on food intake and motivation. We found that: (1) agonists increased food intake after central injections while antagonists decreased food intake after central and peripheral injections. (2) We did not find differences between the effects of opioid receptor ligands on standard rodent food intake after injections in the paraventricular hypothalamic nucleus compared to other brain sites. (3) Mu agonists had the strongest orexigenic effect compared to kappa and delta agonists, with the most significant effect on fat intake. (4) Whereas only centrally administered kappa antagonists reduce feeding, peripherally administered antagonists for all opioid receptor subtypes reduce feeding; (5) peripheral administration of antagonists decreases motivation for food regardless of food type. [2] To determine whether in the paraventricular hypothalamic nucleus signaling through kappa opioid receptor mediates the ability of Dynorphin-A1-13 to increase motivation for sucrose. We assessed motivation for sucrose using a progressive ratio schedule in which mice must perform work licks to obtain sucrose. After each sucrose reward obtention, there was a 20 sec time-out period in which licks were recorded, but sucrose could not be obtained. We found that in the paraventricular hypothalamic nucleus and relative to the vehicle: (1) bilateral administration of the kappa opioid antagonist nor-binaltorphimine increased the time to obtain sucrose rewards and decreased sucrose work and time-out licks in the early and middle stages of the sucrose progressive ratio. These effects were also found after subcutaneous administration of nor-binaltorphimine. (2) Unilateral Dynorphin-A1-13 administration increased the time to obtain sucrose rewards and decreased sucrose work licks in the middle and late stages of the sucrose progressive ratio without changing time-out licks. (3) Bilateral Dynorphin-A1-13 administration did not change the time to obtain sucrose rewards. However, it strongly decreased sucrose work licks in the early and middle stages of the sucrose progressive ratio. Also, nor-binaltorphimine did not block Dynorphin-A1-13 effects. Finally, we could not downregulate the expression of the kappa opioid receptor in the paraventricular hypothalamic nucleus and thus, these effects in motivation for sucrose were not performed. [3] To determine whether oxytocin neurons in the paraventricular hypothalamic nucleus express the kappa opioid receptor and whether blocking the kappa opioid receptor activates Oxytocin neurons in this brain region. Published data demonstrated that the kappa opioid receptor is expressed in oxytocin neurons in the paraventricular hypothalamic nucleus. Thus, we assessed whether nor-binaltorphimine activates oxytocin neurons and decreases food intake in mice. We found that nor-binaltorphimine administration in the paraventricular hypothalamic nucleus decreased cafeteria diet intake and abolished the negative correlation between the percentage of active caudal oxytocin neurons and cafeteria diet intake established after vehicle injection. [4] To determine whether activation of the kappa opioid receptor by Dynorphin-A1-13 and orexin receptors by Orexin-A in the paraventricular hypothalamic nucleus have opposing effects on motivation for sucrose. We found that in the paraventricular hypothalamic nucleus and relative to vehicle (1) Unilateral Orexin-A administration increased sucrose work and time-out licks in the middle and late stages of the sucrose progressive ratio without changing the time to obtain sucrose rewards and time-out licks. (2) Orexin-A decreases the demotivational effects of Dynorphin-A1-13.We conclude that signaling through orexin and opioid receptors modulates feeding behaviors and motivation for food. Those effects depend on variables including the dose and administration route of the drug, food tested, and circadian period in which the outcomes were measured. Dynorphin-A1-13 and nor-binaltorphimine in the paraventricular hypothalamic nucleus decreased sucrose work licks, and Orexin-A blocked the demotivational effects of Dynorphin-A1-13 over time. We reasoned that the demotivational effects of nor-binaltorphimine could involve the activation of anorexigenic oxytocin neurons, and the demotivational effects of Dynorphin-A1-13 could be explained by their signaling through other receptors besides the kappa opioid receptor, such as Mu opioid receptor. We conclude that during the active phase of the mice, Orexin-A potentiates the orexigenic mechanisms that are naturally active in mice and increases locomotion and motivation. Consequently, the orexigenic effects of Dynorphin-A1-13 could not be detected because orexigenic mechanisms reached a ceiling effect. Thus, instead of promoting motivation for sucrose, Dynorphin-A1-13 prompts allodynia that decreases feeding and motivated behaviors.
- ItemDesarrollo de un modelo clínico-radiómico de aprendizaje automatizado, para la predicción de respuesta a la quimioterapia neoadyuvante en pacientes con cáncer de mama.(2024) Soto Moreira, Macarena Cecilia; Caprile Etchart, Paola F.; Pontificia Universidad Católica de Chile. Facultad de FísicaEl cáncer de mama es el tipo de cáncer mas frecuente en mujeres. Aunque existen diversas estrategias de tratamiento, no se dispone de herramientas clínicas para poder predecir con certeza la respuesta de cada paciente a distintas opciones de tratamiento, lo cual motiva este estudio. El trabajo realizado consistió en la implementación de una herramienta para la predicción de la respuesta patológica en pacientes con cáncer de mama tratadas con quimioterapia neoadyuvante. Se utilizaron dos cohortes de imágenes de resonancia magnética con peso T1 (T1w) y con contraste (DCE) previas al tratamiento, una para entrenamiento y evaluación de los modelos predictivos, y la otra para validación externa. Estos cohortes se adquirieron de manera independiente de un repositorio abierto y contaban con información sobre el tipo de respuesta patológica de las pacientes a la quimioterapia neoadyuvante. Para el cohorte de entrenamiento, se realizo una segmentación manual por experto y una semiautomática del parénquima de la mama con el tumor y la lesión. Los resultados de ambos métodos fueron comparados usando el coeficiente de Dice . El cohorte de validación externa incluía la segmentación del tumor realizada por un experto. A partir del proceso de segmentación de tejidos, se extrajeron y analizaron características radiómicas de los volúmenes de interés. Se busco determinar que características presentaban diferencias estadísticamente significativas entre pacientes con respuesta completa y no completa. Se construyeron modelos de aprendizaje automatizado utilizando diferentes clasificadores, incluyendo regresión logística, arboles de decisión, maquinas desoporte vectorial, clasificador bayesiano y vecinos cercanos, con el objetivo de predecir la respuesta tumoral. El desempeño de los modelos se evaluó mediante una validación cruzada con k=2 y 100 repeticiones. Al comparar la segmentación manual y semiautomática del tumor se obtuvo un valor promedio del coeficiente de Dice de 0.635 [0.402-0.857]. Se identificaron un total de 16 características con diferencias significativas, las cuales incluyeron características radiómicas, cinéticas y de subtipo molecular. Estas características no presentaron correlaciones significativas entre sí y obteniéndose un valor máximo de AUC de 0.825. Del análisis de rendimiento de los modelos multivariados, se encontró que el modelo basado en el algoritmo de Vecinos Cercanos (KNN), utilizando la combinación de características: Correlation (Lesión), Skewness (Lesión), Idmn (Lesión), Kurtosis (Peak Enhancement), Kurtosis (Wash-In-Slope) y HER2, presentó el mejor rendimiento en términos de la métrica AUC, obteniéndose un valor de 0.951 [0.890-0.994] para el entrenamiento y en la validacion interna AUC de 0.984. En la validación externa del algoritmo con un cohorte independiente de 81 pacientes, se obtuvo un valor de AUC de 0.955 en la predicción de la respuesta tumoral. Estos resultados demuestran la capacidad de generalización y reproducción del modelo en un conjunto de pacientes independiente.
- ItemDiseño, síntesis, estudios biológicos e in silico de nuevos derivados purínicos trisustituidos como potenciales inhibidores de tirosina quinasas de importancia terapéutica en la leucemia(2024) Delgado Aguilar, Thalía; Salas Sánchez, Cristián Osvaldo; Pontificia Universidad Católica de Chile. Facultad de Química y FarmaciaEn la actualidad, el tratamiento de la leucemia se centra en inhibir vías de señalización relacionadas con la proliferación descontrolada y supervivencia de células cancerosas de origen hematopoyético. Esta estrategia quimioterapéutica está dirigida a inhibir ciertas proteínas Tirosina Quinasas (TK), como Bcr-Abl, BTK y FLT3-ITD. Es por ello por lo que, inhibidores de estas proteínas (TKIs) son fármacos de gran utilidad clínica, pero debido a diversos mecanismos de resistencia y efectos adversos, se requiere contar con nuevas alternativas más eficaces y seguras.En este contexto, esta tesis se logró sintetizar un total de 47 nuevos derivados purínicos. Dentro de los compuestos con mayor potencia inhibitoria en Bcr-Abl, se destacan 7l, 7m, 9c y 10b. Estos presentaron valores de IC50 en el rango nanomolar (4,6 – 14 nM) y fueron más potentes que los fármacos imatinib y nilotinib (IC50 = 327 y 47 nM). Para BTK, los derivados más potentes fueron 9b y 10i (IC50= 0,19 y 0,20 μM) y para FLT3-ITD, 7b y 10e con valores de IC50 de 0,15 y 0,16 μM respectivamente. Los estudios de relación estructura-actividad e in silico permitieron explicar las diferencias de potencia y selectividad en estas quinasas, revelándose información química valiosa en términos de patrones de interacciones relevantes en los sitios de unión y las sustituciones del anillo de purina. Para los ensayos de citotoxicidad en líneas celulares relacionadas con cada una de las quinasas se encontró que en el caso de Bcr-Abl, el compuesto 9c fue el más citotóxico en células K562 (GI50 = 0,59 μM); para BTK, fue el derivado 10i en células Ramos (GI50 = 1,35 μM) y para FLT3-ITD, fue el derivado 10b en células MV4-11 (GI50 = 0,55 μM). Así mismo, considerando que los mejores resultados biológicos de la tesis fueron para Bcr-Abl, se evidenció que, 9c también inhibe a esta quinasa en fosforilaciones rio abajo en células K562 a concentraciones más bajas que imatinib. Por último, cabe destacar que, además 10b, mostró una elevada citotoxicidad en cuatro líneas celulares de leucemia mieloide crónica (GI50 entre 0,7 y 1,3 μM). Por otro lado en un panel de subclones de células KCL22, para las células B8 (Bcr-AblT315I) y F4 (Bcr-AblE255K), 10b fue más eficaz que imatinib y nilotinib en la inhibición del crecimiento de estas y en B10 (Bcr-AblY253H) solo fue más eficaz que imatinib. Estos últimos resultados son muy relevantes sobre todo para Bcr-AblT315I que está relacionada con la resistencia de TKIs y con ello su falta de eficacia clínica. Por lo tanto, a partir de todos los resultados generados en esta tesis, se puede concluir que, gracias al diseño basado en ligandos y nuestros resultados previos, se logró obtener un conjunto de nuevos y potentes inhibidores purínicos para estas tres quinasas, demostrado a través de ensayos bioquímicos e in vitro usados en el desarrollo de nuevos fármacos para la leucemia. De esta forma, a lo menos dos de estos derivados purínicos podrían ser candidatos para los siguientes ensayos in vivo.

Two related studies are involved in the thesis. One of the studies is aimed to resolve the discrepancies between the Triangular Body-Cover Model (TBCM) of vocal folds and clinical data. The study explores theimpact of physical properties, particularly the attenuation factor of the vocal tract, in accurately replicating clinical data, offering insights into the optimization of synthetic voice models for more accurate clinical representation. The second study introduces a novel application of a Probabilistic Bayesian Neural Network (PBNN) for estimating vocal function variables such as subglottal pressure,vocal fold contact pressure, and muscle activation variables, which are challenging to measure in ambulatory settings. The PBNN is trained on both synthetic and clinical data, demonstrating strong performance in predicting these variables with accurate es timations and narrow confidence intervals in synthetic contexts. In contrast, clinical contexts, incorporating transfer learning, present wider, more realistic confidence in tervals due to the inherent variability in human phonation. Furthermore, an observable correlation between prediction errors and both aleatoric and epistemic uncertaintieshighlights the ability of the network to forecast inaccuracies. Increased uncertainty at points of non-linear behavior, especially at higher subglottal pressures, suggests the need for improved input features to capture these nonlinear effects, indicating avenues for future research to enhance measurement fidelity.

Facultad de Agronomía y Sistemas NaturalesEl trigo es una de las especies vegetales más antiguas cultivadas por el ser humano siendo un elemento esencial en la dieta de la mayoría de los habitantes del planeta. El trigo candeal (Triticum turgidum L. ssp. durum) representa solo una fracción de la producción total de trigo en el mundo, sin embargo, el trigo candeal chileno ha presentado una alta calidad y rendimiento que lo posiciona excepcionalmente en la industria agrícola. El cultivo de trigo candeal se ha visto históricamente afectado por las condiciones de sequía, comprometiendo no solo el desarrollo del cultivo sino que también su rendimiento. Se realizó un análisis genético de dos variedades de trigo con índices de tolerancia a la sequía contrastantes, con el objetivo de identificar genes asociados a una mayor tolerancia al estrés hídrico. Para ello se llevó a cabo un estudio transcriptómico basado en RNA-seq, que, acompañado con el uso de herramientas bioinformáticas, logró determinar los las funciones biológicas enriquecidas y los genes diferencialmente expresados de cada genotipo. Se reportó que, dentro de las funciones biológicas enriquecidas en condiciones de estrés hídrico, encontramos la síntesis de fitohormonas como ácido salicílico (SA) y ácido abscísico (ABA) y además la síntesis de metabolitos secundarios. Adicionalmente, a partir de los genes diferencialmente expresados del genotipo tolerante logramos identificar alta expresión de genes asociados a tolerancia a la sequía tales como AGAL1, RAB18, PLD y RCI3. Finalmente, cabe destacar que dentro del estudio de expresión diferencial encontramos genes no identificados que pueden ser de gran relevancia, abriendo la puerta para futuras investigaciones.
- ItemGuided internet-based intervention for postpartum depression symptoms: Development and feasibility trial(2024) Franco Vivanco, Pamela Verónica; Olhaberry Huber, Marcia; Pontificia Universidad Católica de Chile. Escuela de PsicologíaPostpartum depression (PPD) represents a significant concern in Chile due to its high prevalence and considerable treatment gap. Amidst this challenge, the prevalent use of digital technologies and parenting applications offers new avenues for delivering Internet-based psychological interventions (IBIs), which have shown effectiveness in mitigating depression symptoms and hold potential for addressing PPD. This doctoral thesis project embarked on developing and assessing an IBI tailored for PPD, named “Mamá, te entiendo” (“Mom, I get you”). This 8-week guided intervention, aiming to diminish depressive symptoms among postpartum women, is rooted in cognitive-behavioral therapy, incorporating insights from mentalization and attachment theories, and delivered through a web app. Developed in line with the CeHRes Roadmap framework, this human-centered approach involved a systematic review, online surveys, focus groups, usability tests, interviews, and a technical pilot for its creation. To assess the intervention’s feasibility, acceptability, and preliminary efficacy, a pilot randomized controlled trial with 65 postpartum women diagnosed with minor to major depression was conducted. Participants were allocated to either the intervention or a waitlist control group, with assessments at the baseline, post-intervention (8 weeks), and one-month post-intervention (12 weeks). This mixed-methods approach combined quantitative and qualitative analyses to gain insights into participant experiences. Primary outcomes focused on feasibility and acceptability measures like recruitment rates, attrition, and participant satisfaction and engagement. Secondary outcomes examined depression symptoms, perceived social support, mother-infant bonding, and maternal self-efficacy. “Mamá, te entiendo” showed promising feasibility and acceptability, characterized by high satisfaction and engagement levels, although adherence posed a challenge. Participants highlighted several strengths: emotional support, beneficial content, practical exercises, empathetic e-coach feedback, and a user-friendly interface. However, some faced barriers like extensive texts, difficulty tracking progress, and time constraints. Initial efficacy findings indicated no significant differences across outcomes between the intervention and control groups. Nonetheless, both groups experienced improvements in PPD symptoms, mother-infant bonding, and maternal self-efficacy, which could be attributed to spontaneous recovery or concurrent mental health treatments. This study marks the first attempt in Chile to evaluate a psychological IBI for PPD symptoms, underscoring the potential and acceptability of technology-driven mental health solutions.
- ItemIncorporando dinámicas de no aparición en la programación de sesiones de quimioterapia(2024) Lozano Moore, Francisco José; Cataldo, Alejandro J.; Pontificia Universidad Católica de Chile. Escuela de IngenieríaCancer is one of the leading causes of death worldwide and the demand for chemotherapy treatments is growing faster than the capacity health centers have to provide them. Consequently, it is imperative to identify efficient resource management practices for the inter-day scheduling of chemotherapy sessions. We approach the inter-day chemotherapy scheduling problem through a three-step solution approach. First, we reclaim future resources tied to treatments disrupted by patient no-shows. Second, we identify patients for whom current wait times can be reduced. Third, we employ a proactive scheduling policy obtained through the resolution of a Markov decision process via approximate dynamic programming techniques. Using data from the UC CHRISTUS Health Network, we present a case study that demonstrates the impact of no-shows on health systems and the potential benefits that can be obtained from the use the proposed solution approach. From the results, we can conclude that our approach leads to substantial improvements in multiple aspects of outpatient chemotherapy operations. Specifically, the proposed approach achieves a discounted operational cost reduction of 22.3% and a 32.6% reduction in average waiting times when compared with existing algorithms. The results allow us to conclude that there are multiple benefits associated with incorporating no-shows into the problem’s resolution and that the proposed solution approach not only reduces discounted operational costs, but also improves the level of service offered to patients.
- ItemInhibidores de CDK para personas con cáncer de mama: una revisión sistemática de la efectividad de una Droga Oncológica de Alto Costo en Chile(2023) Aedo Bahamondes, Fabiola; Cerda, Jaime; Pontificia Universidad Católica de Chile. Escuela de MedicinaIntroducción: El cáncer de mama, caracterizado por un crecimiento celular descontrolado en tejidos mamarios, presenta diversas formas moleculares que influyen en las opciones de tratamiento. Los inhibidores de CDK han emergido como terapias efectivas para el cáncer de mama avanzado, y en Chile destaca la fulvestrant combinado con palbociclib como una terapia cubierta por el Comité de Drogas de Alto Costo (DAC) desde 2019. El proceso de toma de decisiones del DAC es aún desconocido, lo que motiva la necesidad de explorar estas terapias desde la evidencia. En este contexto, se plantea la pregunta: ¿En pacientes con cáncer de mama avanzado, el uso de inhibidores de CDK asociado a terapia endocrina, en comparación al uso de solamente terapia endocrina, se asocia a mayor sobrevida global, mayor calidad de vida y menores efectos adversos serios?. Objetivos: Evaluar la efectividad del uso de inhibidores de CDK en combinación con terapia endocrina, en comparación con el uso exclusivo de terapia endocrina, tiene un impacto en la sobrevida global, la calidad de vida y los eventos adversos serios, en el tratamiento del cáncer de mama avanzado. Métodos: Se realizó una revisión sistemática de acuerdo con las directrices PRISMA, la estrategia de búsqueda se llevó a cabo en CENTRAL, MEDLINE y EMBASE, se realizaron los análisis por tipo de medicamento y se aplicó la metodología GRADE para evaluar la certeza de la evidencia, se aplicó la escala para evaluar la credibilidad de los análisis de modificación del efecto (ICEMAN) y se presentaron los resultados en una tabla de resumen de hallazgos. Resultados: Se identificaron 11 ensayos clínicos que compararon el uso de ribociclib, palbociclib o abemaciclib con hormonoterapia en comparación con hormonoterapia. Los resultados del análisis indican que con baja certeza de la evidencia los inhibidores de CDK asociados a terapia hormonal podrían efectos positivos sobre la sobrevida global y calidad de vida. Sin embargo, podrían aumentar los eventos adversos serios. Los hallazgos de este trabajo se clasifican de baja credibilidad según la aplicación de la herramienta ICEMAN. Discusión: Las evaluaciones de tecnologías sanitarias (ETESA) como parte del sistema de salud, respaldan el proceso de toma de decisiones y rinden cuenta a la sociedad. Para establecer metodologías sistemáticas de trabajo, Organizaciones importantes como la Organización Mundial de la Salud y la Colaboración Cochrane adoptaron el método GRADE para evaluar la calidad de la evidencia y la fuerza de las recomendaciones en salud, mejorando la trazabilidad y la transparencia. Sin embargo, para poder formular una recomendación se requiere considerar más aspectos, como el equilibrio entre beneficios y riesgos, las preferencias de los pacientes y las consideraciones de recursos. Conclusión: Los resultados obtenidos proporcionan ciertas pistas sobre la elección de los medicamentos DAC, aunque no son suficientes para comprender plenamente la decisión. Este proceso requiere abordar otros aspectos que dan forma a la ETESA.
- ItemOCRL1 Regulates the endocytic trafficking of APOER2 and reelin signaling(2024) Fuentealba Perez, Luz Maria; Marzolo Canales, Maria Paz; Pontificia Universidad Católica de Chile. Facultad de Ciencias BológicasLowe Syndrome is a severe disorder characterized by renal, ocular, and neurological signs. Mutations in the gene encoding the OCRL1 phosphatase are recognized as responsible for this syndrome. OCRL1 targets membrane phospholipids called phosphoinositides, with PI(4,5)P2 being the most common. The functions of OCRL1 are therefore associated with the regulation of intracellular trafficking and protein sorting. Mutations in OCRL1 affect different domains of the protein, both in enzymatic activity and in protein-binding domains, making it very difficult to describe a specific mechanism to explain this syndrome. Little is known about the nervous system, but neurological problems such as intellectual disability, behavioral disorders, and in some cases repetitive and stereotyped behaviors have been described. Interestingly, Reelin has functions in the development of the central nervous system, memory, and learning, and it signals by binding to membrane receptors of the LRP family, such as ApoER2/LRP8 and VLDLR, with ApoER2 being the most relevant. ApoER2 is internalized and recycled, and its intracellular trafficking is important for its function, as inhibiting its recycling significantly reduces Reelin signaling. Therefore, our study aims to elucidate whether the effects of OCRL1 on ApoER2 trafficking are reflected in a disturbance in Reelin signaling and, with this, to explain part of the neurological damage that occurs in Lowe Syndrome. Here, we show that ApoER2 passes through OCRL1-positive compartments and, furthermore, that the absence of OCRL1 disrupts the normal trafficking of ApoER2, affecting its availability at the cell surface and decreasing its half-life. Moreover, we detected a dysfunction in the signaling triggered by Reelin in our LS neuronal model. Thus, our study suggests that the disruption of the ApoER2/Reelin signaling pathway due to OCRL1 dysfunction may be a contributing factor to the neurological manifestations observed in Lowe Syndrome patients.
- ItemParental personality traits and emotion regulation difficulties: their relationship with early triadic interactions and infant’s socioemotional development(2024) Muzard Costa, Antonia María; Olhaberry Huber, Marcia; Pontificia Universidad Católica de Chile. Escuela de PsicologíaBackground: socioemotional development is a pivotal aspect of human life, particularly during infancy, and is intricately woven within the personal and interpersonal dynamics of caregiver- infant interactions. This longitudinal study represents a pioneering exploration into the associations between mothers’ and fathers’ personality traits, emotion regulation difficulties, triadic interactions, and infants’ socioemotional development during the perinatal period. Objective: The primary objective was to investigate the complex interplay between parental personality traits and emotion regulation difficulties in mother-father-baby triads with its impact on early triadic interactions and infants’ socioemotional development. Methods: In this non-experimental, longitudinal study, 55 Chilean mother-father-baby triads were assessed from pregnancy until infant’s sixth month after birth. Additionally, parental levels of personality functioning, relationship satisfaction, depressive symptomatology and childhood adverse experiences were considered as potential covariables. Results: Findings revealed that maternal and paternal attributes have particular predictive values and exert distinct influences on both triadic interactions as well as infant’s socioemotional development from pregnancy until infants’ 6-months postpartum. Notably, maternal emotionality and paternal agreeableness, as well as both significant caregiver’s emotion regulation difficulties emerged as significant factors when fostering collaborative triadic interactions and enhancing infant’s socioemotional development. Additionally, when cofounding variables were included, maternal depressive symptomatology emerged as a significant predictor of infant’s socioemotional difficulties at 6-months postpartum. Moreover, the study highlights the moderating role of triadic interactions, emphasizing their significance in influencing the relationship between maternal emotion regulation difficulties and infant’s socioemotional development. Discussion: This research stresses the need for a comprehensive longitudinal perspective in understanding the impact of significant caregivers’ characteristics upon early triadic interactions and infant’s socioemotional development. Therefore, it acknowledges the unique dynamics of the perinatal period, while considering the individualities within each family triad which is crucial for unraveling the complexities inherent in the intricate fabric of family relationships and infant’s socioemotional development.
- ItemPhysics-informed neural networks approach to predict heart displacement field and in-vivo myofiber strain(2024) Solís Paredes, Rodrigo Ignacio; Sahli Costabal, Francisco; Pontificia Universidad Católica de Chile. Escuela de IngenieríaLa deformación en la dirección de las fibras del corazón, llamada myofiber strain ha surgido como una métrica del desempeño mecánico del tejido del corazón que considera su microestructura, a diferencia de las métricas tradicionales: deformación longitudinal, circunferencial y radial, que son calculadas de una forma meramente geométrica. Predicciones certeras del myofiber strain entrega resultados prometedores para predecir fallas cardíacas. Combinándolo con un campo de desplazamiento continuo del corazón, se abren oportunidades para una mejor predicción de la función cardíaca y posibles diagnósticos. Esta tesis analiza el desempeño y la implementación de una red neuronal informada por modelos físicos considerando una descripción de la topología de los datos dada por las funciones propias del operador de Laplace y una descripción de la energía hiperelástica del tejido cardíaco del ventrículo izquierdo mediante un modelo de active strain para predecir un campo de desplazamiento continuo y myofiber strain. Esto usualmente se ha logrado usando el método de elementos finitos, pero en este trabajo se va a demostrar que las redes neuronales tienen mejores resultados. Este trabajo usa un campo de desplazamiento obtenido con imágenes DENSE y las direcciones de las fibras del corazón obtenidas con cDTI, las cuales son imágenes de resonancia magnética. Los resultados experimentales prueban que la red neural informada por un modelo físico entrega mejores resultados que los obtenido con elementos finitos. Adicionalmente es capaz de predecir valores para la deformación en la dirección de las fibras del corazón dentro de los valores presentes en la literatura. Las imágenes de DENSE tienen un tiempo largo de adquisición, haciéndolo un procedimiento clínicamente poco aplicable, sin embargo, la red neuronal propuesta entrega excelentes resultados considerando una cantidad reducida de datos de DENSE, permitiendo bajar el tiempo de adquisición para estas imágenes y entregando resultados prometedores para su aplicabilidad clínica.
