- Item"Cambia la vida de un día para otro”: Experiencias de madres y padres al cuidado personal de un hijo/a hospitalizado por cáncer(2023) Concha Méndez, Camila; Vega Vega, Paula; Vargas Celis, IvonneObjetivo: Comprender el significado que atribuyen los padres y madres a la experiencia de estar junto a su hijo hospitalizado en una unidad de oncología de un hospital público de Chile. Metodología: Paradigma de investigación interpretativo, enfoque fenomenológico hermenéutico de Van Manen. Se realizó una entrevista en profundidad a 10 participantes y las narrativas se analizaron mediante un análisis temático con enfoque selectivo de lectura. El artículo fue aprobado por el comité de ética de la Pontificia Universidad Católica de Chile. Resultados: La experiencia de hospitalización significó “cambiar la vida de un día para otro”; este resultado se conformó por dos estructuras: 1) “Cuando te dan el diagnóstico se te derrumba el mundo”, que contiene momentos difíciles como la hospitalización en que reciben el diagnóstico, los momentos en que el hijo/a sufre, las situaciones de riesgo vital, el cansancio de las madres y padres y los cambios que afectan a la familia, y 2) “Recibir una luz de ayuda durante la hospitalización”, que describe la importancia de un espacio físico tranquilo, de un equipo de salud con un trato cariñoso y de compartir experiencias con otras madres y padres. Conclusiones: La hospitalización representa una transformación en el mundo vida de los padres y madres, es vital que el equipo de salud atienda las necesidades físicas y emocionales que derivan de este cambio en el espacio y relaciones que estos vivencian.
- ItemChildren’s right to play in Chilean hospitals: A forgotten right?—A qualitative study protocol(2025) Glos Aleksandra, Malgorzata; Santana López, Alejandra; Vargas Celis, Ivonne; Sanhueza Díaz, Lilian; Quezada, ConstanzaBackground Despite being a recognised human right (art. 31 of the United Nations Convention on the Rights of the Child), children’s right to play is still insufficiently researched, understood and, above all, implemented. In Chile, the National Child Health Programme recognises the importance of this right for the hospitalised children but makes its provision dependent on the hospital’s capacity. We therefore hypothesise that the provision of children’s right to play in Chilean hospital settings will be irregular, varying from institution to institution, possibly reflecting the existing socio-economic inequalities in the country, thereby leaving much room for improvement. Aims This study aims to collect qualitative data on the institutional arrangements and opportunities for children’s play in public hospitals in Chile. Specifically, our goal is to (a) interpret the conditions and opportunities for play that Chilean public hospitals provide to children and adolescents; (b) interpret institutional arrangements and professional experiences of children’s play in those institutions; (c) identify factors that favour and/or limit opportunities for children’s play in the Chilean hospital setting. Methods This is exploratory qualitative research, combining multiple and instrumental case study with ethnographic research. Its data collection techniques are semi-structured interviews with professionals in hospital settings (supported by a self-assessment questionnaire), and non-participant observations of hospital playrooms (and/or other play spaces). Conclusion This will be the first study to investigate children’s right to play in Chilean hospitals, with a particular focus on the extent of its implementation, its understanding among health professionals, as well as existing limitations and opportunities for its development.

This confidence would mediate between the management of the patient’s chronic conditions and their effects. Objective: To identify the effect of confidence in health provider/professional-patient relation in different health contexts. Methods: It was carried out a critical review of related literature from June to August, 2017 in order to understand the role of confidence in health provider-patient relation, and its relation with the results in health. The initial search was with the keywords: ´´confidence´´, ´´physician´´, health provider´´, ´´patient´´ and their equivalents in English and French, in the electronic databases JSTOR (Philosophy), Race Relations Abstracts, SocIndex with Full Text, Social Science Database, PubMed Central, CINAHHL, Nursing & Allied Health Database, Web of Science, Academic Search Complete and ProQuest Central. There were carried out different search strategies with the keywords and the boolean operator ´´AND´´. Conclusions: Confidence is a variable of the patient-health provider relation that has a strategic role in therapeutic processes. Then, it is important to consider it as a part of health care, specially as a point of collaboration and commitment of the patient with his/her health. Thus, it is necessary to create instruments that measure this variable in accordance with the cultural characteristics of Latin American countries and this can be extended to all having a care role in the direct contact with patients.

- ItemMetodología activa en el Estudio de Caso para desarrollo del pensamiento crítico y sentido ético(2018) Vargas Celis, Ivonne; González Briones, Ximena María; Navarrete, T.Introducción: Un modelo centrado en la enseñanza-aprendizaje supone un cambio en la docencia universitaria, esto implica el uso de metodologías activas con enfoque en el desarrollo de habilidades como el pensamiento crítico (PC) y sentido ético (SE). Objetivo: Evaluar el impacto de la implementación del estudio de caso (EC) en el desarrollo de habilidades de PC y SE en estudiantes de enfermería. Métodos: Se realizó una intervención longitudinal en una muestra de 103 estudiantes, se aplicó la prueba estadística ANOVA. Resultados: Se evaluó el desarrollo de PC al comparar los resultados obtenidos por los estudiantes en tres evaluaciones, se logró evidenciar un progreso en el desarrollo de las habilidades de PC, y SE. Con el análisis de varianza se estableció que el valor del estadístico F fue mayor al crítico (valor p < 0,05), para un nivel de confianza del 95%, se puede rechazar la hipótesis nula. Discusión: La implementación de la metodología contribuyó favorablemente en el desarrollo de las habilidades PC y SE en los estudiantes, lo que les permitió identificar y resolver problemas, al fundamentar sus decisiones. Además, fomentó el aprendizaje colaborativo y el respeto, entre pares. Conclusiones: La metodología EC promovió un aprendizaje activo y un cambio en la cultura del aula. Si bien, implementar la metodología de EC favoreció el desarrollo de habilidades de PC y SE en los estudiantes de enfermería, es necesario seguir investigando sobre factores que mejoren el proceso de enseñanza-aprendizaje con un enfoque activo.
- ItemMoral Distress, Sign of Ethical Issues in the Practice of Oncology Nursing: Literature Review(Wiley, 2019) Vargas Celis, Ivonne; Concha Méndez, CamilaTo describe the factors that influence the emergence of moral distress in oncology nurses. Methodology: integrative review of the literature whose search will be performed in Web of Science databases, CINAHL (EBSCO), Scielo, Proquest, Pubmed and PsycInfo using the keywords moral distress and oncology nursing and their similes in Spanish, without restriction for years of publication until 2017. Results: The results of this review were grouped into three groups of factors each with subgroups: 1) Organizational factors: ethical climate, evasive culture and resources for ethics delivered by the organization. 2) Particular clinical situations: pain management, information delivery, futile treatment, and assistance to the patient and their family in the process of death, 3) Interpersonal relationships: poor communication, power relations, trust in the team’s competence. Conclusion: the three factors described are triggers of moral distress in oncology nurses. When there are ethical problems and the nurses do not participate in the deliberation process, these problems can be hidden and be normalized, which can diminish the moral sensitivity of the professionals, as well as the possibility of acting as moral agents.

At the clinical level, the issues are the fulfillment of medical care demand with adequate health care teams, infrastructure, and supplies, and to cover critical care demands that surpass the available resources. Therefore, ethically correct approaches are required for the allocation of life sustaining resources. There are recommendations for the allocating life support during disasters based on multiple considerations, including ethical ones. However, the ethical criteria of existing guidelines are variable. Ethical principles usually considered are saving the greatest number of lives, saving the greatest number of years of life and the principle of the life cycle or the goal to give each individual equal opportunity to live through the various phases of life. However, the centrality of the human being and the search for the common good should be considered. Knowledge of public perspectives and moral benchmarks on these issues is essential. A successful assignment effort will require everyone's trust and cooperation. Decision making should be planned and discussed in advance, since in-depth deliberation will be extremely complex during the disaster. Our goal is to help the health care teams to wisely allocate resources in shortage periods.

However, despite the countless benefits of play for children’s wellbeing and development, it is often overlooked in public policies, and especially so in the situation of political conflict and social unrest.Methods: Drawing from the children’s rights framework, this qualitative study explores the role of play for the psychological recovery and social reintegration of children affected by political and social violence. It does so within the wider framework of peace studies and includes the exploration of children’s conceptions of peace and the question of how play can contribute to promoting a culture of peace in violence-stricken societies. Through play-based focus groups with child migrants and refugees in Chile, we explored their own views on the therapeutic and peace-building potential of play as a response to selfreportedproblems. Results: The children in our study told a dual story: a story of trauma and violence, and another one about hope, friendship, and resilience, all of whichwere greatly facilitated by play.Conclusion: For that reason, as representatives of children’s vulnerable voices,we advocate for the greater and more systematic provision of children’s right toplay in conflict zones and its inclusion in peace-building programs.

- ItemThe decision making process in a pediatric patient with Leukemia: ethical issues in Chilean case.(2019) Vargas Celis, Ivonne; Pavéz, TiareIn health care, ethical issues coexist with generally inevitable clinical problems. It is necessary that health professionals have the tools to conduct proper analysis of these problems for accurate decision-making. A traditional therapeutic relationship is made up of two cooperative parts via the decision-making process, whose objectives are to cure and care. In the pediatric environment, due to the legal implications of the child, this process may be complicated because the parents have their own desires and values in relation to the health of their child, which may be in conflict with the wishes of the child themselves. Jonsen, Siegler and Winslade in their book Clinical Ethics developed a decision-making model to analyze cases of medical ethics known as the ‘four quadrants’. This method has as a main function, to analyze the facts and values of a clinical situation with casuistic approach that considers relevant circumstances and the specific context in which decision-making is developed. This article will show a pediatric patient with acute lymphoblastic leukemia, posing the question, is it ethically correct to respect the decision of a teenager to abandon treatment, despite having possibility of cure?
- ItemVoluntades anticipadas: desafíos éticos en el cuidado del paciente(2019) Forero Villalobos, Jenny Johanna; Vargas Celis, Ivonne; Bernales Silva, Margarita MariaLas voluntades anticipadas en salud tienen como finalidad que la persona manifieste de manera anticipada su voluntad sobre los cuidados y el tratamiento de su salud, lo cual le permite expresar personalmente y de forma previa sus preferencias. Esta revisión de la literatura tiene por objetivo describir el concepto y la estructura de las voluntades anticipadas, así como los aspectos éticos involucrados durante el cuidado del paciente. Con las palabras clave “Advanced Health Care Directive” AND “Ethical Implication” y sus símiles en español (voluntades anticipadas, implicaciones éticas), se revisaron cinco bases de datos: ProQuest, Philosophy (JSTOR), PubMed, Web of Science y SciELO; las publicaciones se agruparon entre los años 2010 y 2018. Se obtuvieron 31 artículos de los cuales se realizó una lectura crítica. Los resultados de esta revisión fueron agrupados en las siguientes categorías: concepto, estructura, situaciones clínicas donde se aplican las voluntades anticipadas, fortalezas y limitaciones; así como los aspectos éticos involucrados. Actualmente, son más comunes las vivencias relacionadas con las situaciones al final de la vida, donde la persona pierde su capacidad de decidir y no puede manifestar sus deseos, por lo que es imposible conocer su voluntad. Gran parte de los profesionales de la salud no tienen capacitación adecuada sobre el desarrollo y la aplicación de las voluntades anticipadas, escenario que se convierte en una oportunidad para la investigación y profundización sobre el tema. Las voluntades anticipadas son una herramienta que proporciona al equipo de salud información fidedigna de los valores y deseos del paciente, por lo que es importante capacitar a estos profesionales para brindar una atención respetuosa y de calidad.
